If both parents in Malaysia: Declaration of Parent/Guardian for Minor’s EC in Annexure-‘D’ if they don’t have marriage certificate declaration in Annexure-‘H’ & Appendix-12
If one parent is in Malaysia and other parent is in India/abroad: Annexure Dsigned by the parent in Malaysia; and Annexure D signed by other parent in India/abroad duly attested by Indian Mission or notarized & apostilled in India https://mea.gov.in/apostille.htm

- Please choose country Malaysia carefully
- Register & keep credentials safely (It will also help in tracking the process)
- Login & fill up the form
- Please fill in the form exactly as per the passport, if there is no change in any fields. Any change in spelling without documentary proof may lead to undesired delay in issuance.

- Please take a printout of the duly filled online application form after successful submission and affix the photograph. The form must be signed by the applicant. (In case of a minor applicant, the form must be countersigned by parents/legal guardian) *(Signatures are required to affix before the Submission Officer of BLS International.)

In addition to the above fees for EC, amounts of RM 9 will be charged towards Indian Community Welfare Fund (ICWF) fee and RM 9.50 towards the service fee by the service provider, the BLS International Services Ltd. Additionally 1.5% of the total fee to be paid will be charged on payments made by Debit Card.
